In this article today, we will talk about HCG Trigger Injection. In common language, it is known as Ovulation Injection or Trigger Shot Injection. HCG, which is full form of Human Chorionic Gonadotropin, is a pregnancy hormone which is released from placenta during pregnancy. Its main function is to support early pregnancy. We use HCG in clinical practise in two ways.
- First, for pregnancy hormone support.
- Second, for ovulation in infertility patients. Ovulation is used to release egg from follicle.
In today's video, we will talk about how HCG injection is used in infertility. What is its action? How is ovulation done? And in which infertility patients it is used? HCG hormone is used in infertility patients because its structure is similar to our natural ovulation hormone LH hormone. LH hormone and HCG hormone have similar chemical structure and action.
What is the function of LH hormone?
As soon as egg in our body reaches maturity level, i.e., its size is between 18-20 mm, LH hormone level starts rising in our body. This is known as LH surge. When LH hormone level is so high in our body, there are chemical reactions in egg follicle. As a result, egg finally reaches its maturity level. And ovulation of that egg is done. LH hormone level starts rising in our body and duration of ovulation is 36-48 hours.
When LH hormone peak in our body, after 36-48 hours, naturally our egg is released. And after that, LH hormone level comes down. Similarly, when we give HCG injection, its final maturation and ovulation is done. And when we give HCG injection, its final maturation and ovulation is done. In this way, we use HCG injection for ovulation. HCG injection comes in two forms.
- First is Urinary HCG and
- Second is Recombinant HCG. Recombinant HCG is a synthetic form.
In Urinary HCG, we give 5,000 to 10,000 doses which is intra-muscular. In Recombinant HCG, we give 250 microgramme dose which is subcutaneously. These doses are for infertility patients. In HCG injection, it is important to maintain cold chain.
Timing is also very important. Because we know that when we give HCG injection, after 36-48 hours, ovulation will happen. So, we give HCG injection according to the patients we are using.
Next, we will discuss how HCG injection is used in infertility. In clinical practise, we use HCG injection in three types of patients.
- First is for natural dry patients.
- Second is for IUI patients.
- Third is for IVF patients.
In all these patients, we start giving egg making medicines and through follicle study, we measure the size of follicles.
When these follicles are 18-20 mm in size, means when they mature, at that time, we use HCG injection. As we know that when we give HCG injection, after 36-48 hours, ovulation happens.
- So, the first set of patients who want to give natural dry, we normally advise them to give natural dry after 24-48 hours. Means you have to start natural dry from the next day. This will increase your pregnancy rate.
- The second set of patients is IUI patients. In IUI patients, we plan the IUI after 40 hours of HCG trigger shot. And before IUI, we document the ovulation with ultrasound.
- The third set of patients is for IVF patients. As we know that in IVF, we have to remove the eggs from the body. So, whenever we give trigger shot injection, we remove the eggs from the body for 34-35 hours. After 36 hours, if ovulation happens, these eggs will be released in the body and will be of no use to us. So, in IVF patients, the timing of trigger shot injection is very important. And whenever it is called at the exact time, you have to give this injection at the exact time.

HCG injection is when we give medicines to the patient to make the seed and call the patient for sonography to check whether the seed is ready or not. When the seed is ready, the size of the seed should be at least 18 to 22 mm. Then we call that seed ready.
Sometimes the seed is small, 15 mm or 16 mm, then it is not a ripe seed, it is a raw seed. So, we never apply HCG injection on that seed. When the size of the seed is 18 to 22 mm, then we apply HCG injection.
